You may need a new laser if your getting "Disk Read error" and you have cleaned your laser.

They aren't expensive in the slim ps2, you just need to make sure to either have the retailer desolder the anti-static points or you have to do it when you recieve it.

Do you have a modchip installed?

You may need a new laser. You can follow this guide to change it yourself.
