Are you rebooting after you insert the usb stick?
2 Scenarios I have tried
a) I insert the USB stick in the console whilst in the Dashboard (I expect a system update to appear at this stage but then nothing happens.
b)I insert the USB stick whilst the console if switched off, I turn on the machine (I expect a system update to appear at this stage) and nothing happens and the boot continues to the dashboard.
I think I will format the USB, and try it again. (Scracthes head)
EDIT - Formating and re-installing did nothing either. I think it must be because I installed the smaller update and the system thinks it is fully up to date.
Banned on iX1.5 Liteon. Currently wants to buy a new xbox but does not understand how all this flashing works and will not pay £40 to get this xbox flashed again.

If you have updated with the 8MB release and lost your avatar then simply re-update using this release.
1.) Remove the 8MB $SystemUpdate_9199 folder from your USB stick.
2.) Extract the 83.5MB $SystemUpdate_9199 folder and rename the folder to $SystemUpdate Then transfer to your USB Stick.
3.) Insert the USB Stick into your Xbox 360 console.
4.) You will be automatically prompted to update.

Ok, I'm having real issues updating my 360 from 8498 to 9199. My HDD is crapped out so I cant update online. How am I suppose to install this update with a DVD? I've followed the instructions from the official site with no luck at all. I put in the DVD, it reads it, and tells me it is a "Mixed Media Disc" but nothing happens.
***Update***
Ok, I finally got the system update to happen, except Its informing me I need 128 mb of free space. Do I need the HDD? I'm getting seriously annoyed with this whole ordeal.
