I've searched everywhere and got some decent information, but no one includes any firmware when they post. I have a NEC-3500A with official firmware 2.18 and a PS2 V4 with a no-solder mod. I also have Nero 6.3 and the latest DVD Decryptor while burning at the slowest speed available. I have a couple of successful backups from my friend's burners (models unknown) but I haven't been able to make one myself. Here are some stats on my burner and the media I have currently available (Maxell & Fuji). Both medias will play perfect as DVD movies, but PS2 games on the same media are choppy. They will either:
1. Not Load
2. Load but crash at the menu
3. Skip will playing (especially music)

I do have some +R media, but cannot currently bitset it because of firmware and the V4 PS2 doesn't seem to like it anyways. Please help me out. I'm curious as to what other NEC users have for firmware.

I havent bothered to update my firmware from the 2.16 it came with since it burns media at or above 8x. With DVD ps2 games and swap magic/flip top, I've had middling success, though I've only been playing back up games for about a month. Dragon Quater burned on Maxell +r riconjpn played flawlessly. Shadow Hearts 2 on fujifilm riconjpn +r would sometimes freeze during movies, but i was able to get through the game ok. I've burned and booted others, but those two are the only ones I've played all the way through. My ver 5/6 seems to prefer +r, so thats what i use now, TY 8x +r. I still haven't found a successful way to play cd ps2 games. La Pucelle Tactics and Growlanser Generations are the only ones I've tried though.

Yes I do know this, and yes I have been playing a couple of backups successfully for a while now.

Since you're using official 2.16 FW, I assume that you are not bit-setting the booktype to DVD-ROM. I'll attempt a +R tonight (with default booktype) and get back with you.
