for us and canada I know http://www.consolesource.com is good. I got a v9 and slim pstwo flip top and swap magic. Each set was $47.99 including shipping and arrived in a week.

No but it is really easy to fit. Just do a general search in google on how to fit the cover and thats that. There will be loads of information available

Your fliptop may already have the memory doors and the white disc installed depending on your PS2 version, so just skip those parts.
