DuoX 2 Cromwell blinking red

I have a v1.1 xbox and DuoX 2 Cromwell and I Carefully installed it exactly as shown by:

When I turn the xbox on it reboots 3 times and does the infamous red-and -green blinking and the tray will not eject and there is no A/V output. The LED on the Duox2 chip itself blinks red.
I would like advice as to what to do next. Is the duox2 supposed to blink red, and if not does this mean it has to be flashed?
