This error usually denotes a hardware failure. Read below what the cause of this error is.

http://www.nero.com/en/FAQs_Error_Messages.html#14

14. What do I do if I get a 'Focus servo failure' message?

The "Focus servo failure" message is a hardware error. It can be caused by the following:

Poor media - Try more than one brand of CD-R media to rule this out as cause.
Poor power - The power to your recorder could be fluctuating. If you have an internal recorder, try a different power connector and make sure it's not being shared by any other devices. If you have an external recorder, if possible, remove it from the external chassis and connect it to your system as an internal recorder. If this resolves your issue, you need to replace or repair the power source in your external chassis.
Optical head is dirty, needs alignment, or recorders circuit board has an open coil - please contact your recorder manufacturer to service your unit.

MKM is Verbatim's parent company, Mitsubishi Kagaku Media (and MCC is their parent), but the only MKM codes i've seen are on here - http://forums.afterdawn.com/thread_view.cfm/155431

- and you guessed it there's a similar code on there by.....Memorex

I burned these disks with Decrypter using Read and then Write mode. Do not know why you would make an ISO with Shrink first, this would only compress the movie and all you would need is a single layer disk.

They are expensive, 3 of the Verbatim are 19.88 at WalMart. If I had not wanted to backup my Star Wars boxed set I would still be waiting for them to come down in price. Supposedly TY is going to come out with there dual layer soon, so I will wait before buying anymore.

@JD - i have this in one of the links in my sig -
Quote:
Memorex/CMC et al can't even make single layer discs that work / or that are even consistent in being crap. They fool ppl into buying them by giving a few good burns, but then generate huge amounts of traffic on sites such as this due to the hassle they cause so many ppl. And Memorex/CMC et al being let loose on dual layer media. The mind boggles..

Check out this thread

http://forums.afterdawn.com/thread_view.cfm/194514

You will see in the burn log that the firmware version is A116. Try seeing if you can find an update for your firmware.

i was a bit too anti-AnyDVD a while back, i'm very much more careful how i phrase things these days, as in hindsight i had a tendency to be a bit confrontational.

i just try to lean ppl away from crap, just like you lot have always done i guess. if ppl don't take the advice, they can flame themselves :)

Has'nt this got some tounges wagging. Yes they are Verbatim discs and after a minor hiccup in the beginning of my first DL burning experience I have burnt two more successfully with DVD Decrypter. This time I used Shrink to read (and you can tell shrink that u r using 8.5 G discs so it does not compress)and Decrypter to write. This is when the error occured so I will go back to using Decrypter for the whole burning process and see if all goes well. Just need to check my lottery ticket to afford another disc to (lol :) If not I hope it is not a problem with my burner, don't know if the warranty has run out yet? So thanks for all your help again ppl and will keep you informed of any findings.......

Downloaded the latest firmware the last time I was having problems and that is what I believe fixed it. Burnt a single layer disc today to see if any probs but none occured. Thought if it was a servo tracking error that it may hve shown up on a single layer disc, but no it did not. Maybe it had something to do with the DL disc when it was changing layers????
