Hi there,
Download VSO Inspector v1.1 --> http://www.vso-software.fr/
Put a blank media in your burner, and start VSO Inspector.
You will get all the detail on what's your burner model, what firmware version is on there, and some details on your media, like the MEDIA ID.
From there we can see if you need a firmware update, and if you have poor, medium or good media.
If you burn with Nero or CopyToDVD , provide us with a full log.
There's lots of information in there !

What model litey do you have? What speed are you burning at? Are you bitsetting your +r/rw media (your drive is capable and recommended)? Is your f/w current?
