Duo 2 SE worked only one time after modding - help!

OK, I've recently soldered a Duo 2 SE Modchip into a V7 PS2 with help from my father who is very experienced in soldering. It was still very difficult but in the end, it worked...

...only once, though.

After the chip was inside I tried to play a PSX backup (Kula World) and it booted perfectly and I played for a while.

Then I tried a DVD-R copy of Tekken 5, however, the PS2 refused to play it.
Even worse, it refused to play any other backups as well, even Kula World which worked initially.

Apart from this, the PS2 works flawlessly as far as I can tell, it still plays original games.

So what can I do now? We re-opened the PS2 and looked and poked at everything but the modchip still refuses to do what it's supposed to do.

We think it might probably be broken since first it worked and then not anymore, but why could it suddenly be broken? Also, is there any way to be sure it's the chip and not something else?

I hope you guys can help me out!

if you uninstall the chip (desolder it)
then you would be able to tell if it was just the chip
either that or find out if the chip can be diabled and
try to do that then boot an original game.
Other than that i can't think of any reason why
it would just stop working. I would say it is either the
chip, or something wrong with how it was installed.

How would desoldering the chip help?
The chip can be disabled, but why would I want to do that? Original games work with no problems! It's only the modchip that doesn't work.

EDIT: The company where I bought the chip said I should clean my laser and it's unlikely that the chip is defective. Does that make any sense? I mean, the chip worked for one time and then never again. I doubt the lens got that dirty within a few minutes.
