It could be anything with the info you've given us... try being more specific.
what version ps2 you have? what are you using to boot the games, modchip or swapmagic+fliptop etc? what brand dvd's you using? dvd-r or dvd+r and what max speed? what speed are you trying to burn at with dvd decrypter?
+ how did it fail? do you mean that when you put it in the ps2 it didn't work? or do you mean that the burning process failed?

You still didn't mention what you are using to boot the backups with, swapmagic or modchip?
[edit] and if you have swapmagic, are you using the slidecard or fliptop?

Yep, sorry missed that.
ok, well you said that you are trying to make a backup of a backup.. did the original backup work ok on your ps2?
if the answer is yes it does work ok on your ps2 then my suggestion would be to just use copy dvd with Nero... that works for me.

Yes, you could still do a copy dvd with Nero with one dvd drive.. but forget that for now... I can't be bothered to explain how to do it.
Have you still got the .iso file on your computer?
and when you tried to back up last time did you have any programs running in the background or have the internet turned on?

Yes it does affect it.. when burning remember to turn off all non-essential programs and disconnect from the internet.. it can affect the burn process.
try burning the image again at 4x with DVD Decrypter.

The only thing I can think of to suggest would be to start again from scratch with the original game dvd.

try updating your burners firmware if one is available
and also do what paddymail mentioned
burn at 4X with DVD Decrypter and make sure there is nothing
else running in the system tray or backround like antivirus
software. TDK 8X DVD-R i have used many times and they worked
for me, but that doesn't mean your PS2 will like them.
what kind of mod chip do you have, there may be a special boot
method on how to use it. Have you made any back ups that do work?
What game is it you are trying?
sorry bout all the questions, the more we ask you the easier it
will be to narrow down the possible problems, there are so many
little things it could be.

Did you try getting the .iso off the original game dvd again?
and are you trying the other things we suggested about turning off programs and internet etc?
Other than that all I can think of is that there must be something wrong with the modchip or the tdk media...
Do you just put the copied game straight into the ps2 and it boots it... or is it one of those modchips where it has a disc you use too?
Only other thing I can suggest is to get a small pack of good media like ritek, taiyo yuden or verbatim DVD-R 8x and try burning on them... if it still doesn't work then i'd get your chip looked at.

Are you sure the modchip itself is working? When I first got into this, I got a modchip from a friend, put it in and thought I was making more coaster than I had drinks. Turned out the modchip was bad when I tested with a Swap Magic and everything worked fine.

I can't be sure as you don't even know what your modchip is...
but if you can turn off the modchip then yeah it should work fine.. remember that you need to get a fliptop or slidecard to work with the swapmagic too though... look in my sig about the ps2 hardware.
