Try without sim first from "insert sim" screen. If it doesn't work, insert the original sim and power on and enter the same code again.

EVERYONE THAT IS REMOVING THE SIM AND TRYING THE CODE AND THEN PUTTING THE SIM BACK IN AND TRYING THE CODE AGAIN IS WASTING YOUR ONLY 5 CHANCES. IF IT DOESN'T UNLOCK WITHOUT THE SIM, IT'S NOT GOING TO UNLOCK WITH IT IN.
