The only real problem i can see from the log is here:
*********************************************************************
Media Specific [16..63]:
00 20 10 00 00 00 00 00 - 00 00 03 12 00 00 00 00 ................
00 00 00 00 00 00 00 00 - 00 00 00 00 00 00 00 00 ................
00 00 00 00 00 00 00 00 - 00 00 00 00 00 00 00 00 ................

AND

Status: 0x04 (0x01, SCSI_ERR)
HA-Status 0x00 (0x00, OK)
TA-Status 0x02 (0x01, SCSI_TASTATUS_CHKCOND)
Sense Key: 0x03 (KEY_MEDIUM_ERROR)
*******************************************************************

These two fields suggest that your burner was unable to recognize and properly write to the media you're using. You have a samsung drive, but there was no firmware update available on the site(could be still new).

Here:
*****************
Recorder: TSSTcorp CD/DVDW TS-H552L, Media type: DVD-RW
Disc Manufacturer: RITEKW - 04
***********************************************************

First, try not to use RW media, it's best suited for data storage not movie backups.

Second, "Disc Manufacturer: RITEKW - 04" , RITEK disks are generally considered to be very poor in quality and these (to me) appear to be the cause of your problem. Change the brand of disks you use for your backups to SONY DVD+Rs or Verbatims DVD-Rs and I'm sure you'll have better success.

Keep that write speed at 4x, it's a good thing.

EDIT: Please remove your serial# from your log

also enable your burner dma to on from off. "CdRomPeripheral : TSSTcorp CD/DVDW TS-H552L atapi Port 0 ID 0 DMA: Off". close down any programs running in the background to free up some ram.
