Toshiba -RW problems

One out of every three of my TDK discs will go to 99% format and then I get a "DVD cannot be formatted" message. All have been recorded on five or six times with no problem.

Do not have a Toshiba. Have LiteOn/ilo units and they use + disks
and I use +RW verbatim [The + RW disk is not finalized]
But
A few times when re-using a disk The unit will Erase [format] the disk and the unit says it is recording and while the disk indicates the recording is there It will not play
I would than take the dsik to a PC and erase the disk
The disk now works fine and than seams to work fine with erase in the unit and future use.
This has not happened enough to question the unit and I feel that It may very well be somthing, I did, that the unit did not like.
