May be defective media. What brand are you using?? Try using good quality like verbatins. Burn at a lower speed, at least half of media speed, I use X4, some others use X2.

Hope this helps :)

What speed are you burning at? If the speed is to high, this may be causing your problem. Because you can burn at a high speed and be successful, but you could also burn at a high speed and have problems or not even be able to complete the burn.

What program are you using to burn?

Burn at a slower speed
Update your DVDWriters firmware
Booktype your dvd's into DVDROM if using +R discs

Verbatim is excellent media but maybe you just have a bad batch.

Before you burn play the movie on your pc first, if all is well then burn.

I have Verbatim 16x and am burning at 8x. Should i burn at 4x? I'm using Nero to burn my backups. Why would the second copy always work? Very strange.

I would keep it at X4. You may want to check if there's an update to your disc drive firmware also.

What speed is your drive rated? Burning too slow can sometimes give as many errors as burning too fast...those Verbs should be fine at 8X or higher, I burn mine at 12X. As suggested make sure your firmware is up to date. Also keep your compression in check, your target output size should be 4300, if you have high amounts of compression and you've removed all audio except AC3 5.1 and your subtitles you might need to use Advanced Analysis and DVD Shrinks AEC controls.
