i have tried 4 codes - all bad, so if you are an expert - please help.

I have tried:
#pw+386076015442470+7#, #pw+838 981 055 602 440+7# and two codes, which I belong to other phone.

Fist code, as it looks to me, should work, however, it doen't...
These are settings used for its generation.

DCT4 Codes for IMEI: 353383000913098
Provider: 31020000
U.S.A., T-Mobile
Type: 5, V2 Codes

Let me know what's wrong and what code should I use to unlock the phone.
