Here's something I wrote in another thread:
Requirements:
A Ps2 (any will do I think).
Swap Magic CD ( I don't think the version matters, I use 3.6)
A real ps2 game DVD-5 (High TOC size is a must! Like Devil May Cry or Xenosaga 2).
CD loader (as a bootable disc).
A PS2 DVD backup game.
1. Boot the SM CD version
2. Boot CD loader
3. When CD loader appears, eject CD Loader properly (DON'T USE THE SLIDE CARD OR TOOL METHOD.)
4. Insert the real DVD PS2 game. The screen should load some numbers of the bottom like 4.37GB, when its done reading the disc.
5. Press 'X'
6. Screen will ask you, Boot EA method or something. Now you replace the game with your backup, using the Swap Magic method (I.E Slide card or slim tools).
7. Press 'X' again and the game should boot.
This is the best I can describe the method. As for the items, you'll have to find them yourself. Otherwise its all set. I think you can also use CogSwap or Zoneloader to do it, but I haven't tried it out.
TOC size list:

Bootable CD loader (you must compile it yourself or something):

What you have to do is simple. I have the same problem, but only with games. You laser is basically not adjusted in the right position. What you do before you start it up, manually move the laser right up, push it up softly.
Now try, it should be positioned. If this causes problems when to start a game, adjust it after the swap.
To re-adjust it, what you must do is push it all the way back to the spindle.
Simple. However, if anything, please leave your cover open as it will reset the adjust during game play.

The CD loader method completely works on the PS2 slim. I know because I have a PS2 slim. I guess it the CD loader disc that is causing problems. You might need to find another image or compile one yourself.
