Need help to unlock Nokia 6030

Follwowing are the details

Firmware Version: v 5.24

Nokia: 6030b
IMEI:011125001502626
Network: T-Mobile
Country: USA

Codes Tried

I have attempted the following two codes

#pw+495487614324501+1# ( Entered 2 times )

#pw+485207614324501+1# ( Entered 1 time )

I understand I have only 5 attempts to enter the code and I have exhausted 3.

Please help.
