here is the log, fails at around the same spot every time, any help appreciated

First, edit out your S/N before some steals it! Click on the icon in the upper right hand corner.

Make sure your firmware is updated for your burner.

Make sure your machine is clean. Defrag your HDD and run ccleaner. Here is a link: http://www.ccleaner.com

Burn process failed at 16x (22,160 KB/s)

Way too fast. Rule of thumb. Burn at half the speed of your media. In other words, if your using 16x DVDs, burn at 8x or below. Will eliminate most errors.

Also, mind the quality of your media. Crap media will do this also. Best media to use is Taiyo Yuden or Verbatims, or MIJ. You can use a program to find out who manufactured your disks here: http://dvd.identifier.cdfreaks.com/
