Ok, so last night playing FO3, screen just turns black outta nowhere. I've never had issues with my box before. About a year old, Falcon type.
So as the screen turns black, I get 3 red rings in this position:

They are flashing red AND the center green is flashing GREEN along with them!?! I've seen the vid's of 3 red rings where the middle green power light stays solid.
So I turn it off, and turn it back on...boots up, gets to dashboard and everything seems normal....
But i'm a bit afraid so I turn it off and it's been off since. Anyone knows what 3 Red flashing AND center green flashing means?
Recently with FO3, the 360 sounds like it's working really hard reading the disc, like it's having a hard time reading, but the game plays fine and loads areas and sounds/video fine.
It might not help that a couple days ago my dog grabbed the controller usb charge cable and pulled it making the 360 slide off it's shelf onto the floor a couple days ago....but it fell against the cable plug and not like a direct *slam* into the floor. It was also probably about a 5 inch slide/drop so not an huge shock to the beast that is the 360. :-P
You should play it more after you boot it up. If it doesn't show up again there's no point in worrying about it.
No, though, I've never seen a green flashing center. Maybe your LED's are just a little funky. You should try to find the error code if it happens again. Hold the sync button, press the eject. Count how many are flashing. Press eject again, count. Press agian, count. Press again, count. If all 4 are flashing then that is a 0. You get the error code and can look it up from there.
My cousin has had a couple issue's w/his xbox that i have flashed w/ ixtreme 1.4. The first time it would just shut off immediately after hitting power, and his power brick was flashing green and orange. We couldn't find anything and i told him to unplug everything and leave it for a while, and it began working fine again. Then just recently he was playing (i think usually just gta4) and he got that exact error, like an rrod but the middle light was flashing. I have read that sometimes a power block can be loose and cause an rrod like error. So what i'm thinking is he has a slightly defective power block that gives him errors once in a while, and since yous aid your dog pulled it off the shelf maybe you should check your connections and make sure everything is tight. I did read some ppl getting rrods during FO3 though. I would suggest trying other games that don't require so much of a load on the cpu/gpu first and seeing if you get any problems.
Since it is a falcon though even if it rrods it will still be under warranty. good luck hope this information is helpful.
I talked with M$ support and they said it doesn't matter if center is flashing, still is a hardware fault. Although the guy sounded like a douche and took him about 5 minutes to respond back after telling him about the center ring flashing. So I checked all the cables, did the cache cleaning, (how fast does this happen, it was like over as soon as I hit "x" !?!) and played it again for about 40 minutes, no issues.
1 interesting thing is I reburned the game, but this time I UNCHECKED verify because I left that on, and made sure the "dvd-rom" option was selected because I think it was set to auto before.
Now the dvd doesn't seem to spin up as hard BUT....the game loads slower and when I bring up the pipboy, it sometimes scrolls though my long list of items slowly at times. Other than that, game play is totally smooth, VATS shooting is smooth, no other issues. Kinda strange though but i'm not sure if it's because I did the cache clean (I think) or the reburn. ???
