Yes cios is the same as cIOS38_rev12. It's also the same as cIOS36_rev10, just a different revision.
There isn't a revision 15. Highest one so far is 13b.
A game like Super Mario Galaxy should work in the USB Loader on every revision since 9 (9, 10, 12, 13, and 13b)
Also, I'd make sure that I have the latest revision of USB Loader GX which is 532 http://usbloadergx.koureio.net/downloads/revisions I'd also try adding a game using WBFS manager on your PC.
There are so many changes between different revision. Even the method of installing a game to the hard drive is different between some revisions.
