No, you don't have to patch games to play on iXtreme LT. It is a bad idea to go on Live with wave patched games.
Your issue is probably your media. What type of discs are you using?

Are you checking your images before burning? You can also check the game with ABGX after you burn to confirm that the burner is properly burning the game.
Burning a 2.4x is somewhat of an urban myth. If your media and burner are rated for higher rates, you should burn at higher speeds.
I would double check these discs you are having a problem with in ABGX.

It could be down to the laser calibration data missing.
There has been a few issues like this with LT
A new JF guide came out and there was a bug fix
Bug fix - Calib data not being flashed

I burn at 8x and no issue here with those games listed above. If the HDD install fails at 4%, the 360 is definitely struggling to read the disc and then fails.

I bought a per-owned 360 of ebay a week ago and it could read retail copy of tekken 6 fine and after flashing it with LT it was struggling to read backups. darksiders was working up to the main menu and then taking ages to load and the rest where hanging at a black screen. I replaced the laser from a spare BenQ drive and now all the games work fine.
The laser defiantly works harder to read DVD-R DL so I think you laser is probably weak. try cleaning, POT calibrating or replacing the laser.

After the release of how to dump full LiteOn firmware, it was found that there is calibration data embedded in the firmware. Initial LiteOn extraction methods did not pick up this data. C4 has stated that the calibration data is not accessed by firmware code at all but may be for other processes.
As for ABGX, make sure that you aren't getting any reds (Game CRC corrupt).
Quote: <c4eva> cal data is not accessed by fw code at all but could be used by other processes, its value is still unknown, but since full dumps are now available, included for correctness

It can't read past 82% on the disk. You need to re-burn another game.
I know your Pioneer 216 is rated to 8x, so I would burn at a faster speed.
