Are you on a modded xbox? make sure you have settings set to write and not just verify. Also it might just be a bad copy of the game. do other backups work?
That media isn't good for use with the 360. And you're saying it's flashed with lt. Is it lt or lt1.1? And did you do a full firmware dump, meaning you got your laser calibration data, or did you just do a simple key dump?
Is your burners firmware up to date? If not then update it then burn a disc at 4x.

LT and simple key dump I assume, I only went through the necessary stages. I dont think its my burners firmware, if it plays on a computer anyway? The game is not a new release so it should not need LT 1.1.
Could anyone describe more about the laser calibration data extraction?
