Xbox Backup Creator, but you'll need a 360 drive flashed with 0800 firmware to rip the image to your computer's HDD. The search feature on this site is very useful. There are lots of threads on this topic alone.

Yes. But you'll need to pactch the ISO with SSv2 files to be "Live safe".
