In that case the very first thing to check would be if any of the connection points in any of the ports are bent. This can happen if you plug in something upside down and will stop them all from working. Also if you've ever opened the case and especially if you've ever removed the motherboard, check to see that none of the metal tabs around the port are touching inside.
