Nokia 6101b- unlock not working

When I enter an unlock code I get "Phone Restricted" flashing at me... I dont know if this is because the code didnt work or because my phone is hard locked??

OK... So I did all of that (both codes)... after I entered each code it said "Phone was not restricted"
So I put in my AT&T SIM and it shows full bars and everything and seems to be working great. But when I try to access messages or anything else it says "SIM not ready!"
and then all bars drop and says "Insert SIM card" and starts flashing "Phone restricted" at me again. I have the option to UNDO and enter a restriction code.. but I don't know if I should.

What does that mean? What do I do??

It's a brand new SIM!! Less than 4 days old!
