You must know where you bought the mobile from?
When you start up the mobile it usually says what operator it is.

You had to take the sim out first:
Remove sim first,power on the phone.
After the phone boots up, press #
Press * 3 times fast for (p)
Press * 4 times fast for (w)
Press * 2 times fast for (+)
Verify code before pressing the last #.
"Phone restrictions off", it's unlocked
How many times have you tried in the past?
Your phone is now hard locked and can only be unlocked by a professional.

without the sim card only the two codes you gave me and it says cannot undo restriction. with the sim card I tried more than 5 times

These codes are for the USA if you are not from the USA do not attempt these codes.
#pw+135024153343377+1# or
#pw+254215471225266+7#
Remember you only get 5 attempts then your mobile becomes hardlocked. I advise only 3 attempts in total.
Remove sim first before you attempt the codes. :)
