hello,
Quote: Recorder: <LITE-ON DVDRW SHW-160P6S>Version: PS08
There is a firmware update here
http://forum.rpc1.org/dl_firmware.php?download_id=2175
Quote: Disc Manufacturer: CMC MA - G. AM3
Sense Key: 0x03 (KEY_MEDIUM_ERROR)
Try better media like Taiyo Yuden, Verbatims, note look at the error
Quote: Last possible write address on media: 2298495 (510:46.45, 4489MB)
Last address to be written: 2286975 (508:13.00, 4466MB)
Burning too close to the edge, try changing output size in shrinks preferences.

well if u did everything but change the media then my guess would be that it is more than likely that the media is the cause of the problem.
BTW what media r u using?
Memorex?
Imation?
TDK?
