Could be poor quality media, outdated firmwre, wrong burn speed, multitasking, etc. Could be one or a combination of above. But, until you provide more particulars on your procedures, one can't hazzard a guess...

Hi Granny, here's some tips:
As ZosoIV says:
1) update firmware on burner
2)burn no faster than half speed of disc (best to burn no faster than 4x)
3) use good discs an Taiyo Yudens or Verbatims (made in Japan- MIJ)
4) incorporate fixvts into your algorithm (this is my idea)
5) use imgburn as your burning engine and bookype to dvd-rom if using dvd+r discs
HAPPY BURNING

If you choose to flash, the safest way would be to do it from safe mode, with no discs in the drive that you are flashing, but there are no guarantees. As asaked please post a Nero burn log, which will give most of the information recquired, just copy & paste in a reply post. Program Files / Ahead / Nero / NeroHistory.
System: Microsoft Windows Xp Pro 2002 SP3 / Windows XP Pro X64 Edition. Computer: AMD Athlon(tm) 64x2 Dual Core Processor 4800+ 2.51 GHz. 4.00 GB of RAM.

Hi Granny, I'm not Zoso (actually I quoted him). Firmware is the software that runs your drive (burner). Yes it is downloaded and can be found at the website of the manufacturer of the dvd drive. It is usually in the updates section. You need to know the model of the your drive in order to download the correct update.
