I have test two codes:
#pw+ 41 82 77 13 66 74 77 0 +7# (V1 Code)
#pw+ 40 82 77 13 66 74 77 0 +7# (V2 Code)
the phone is a Nokia 6101 black..
IMEI : 358362000184756
Blocked to: Vodafone Portugal

The codes etowah are providing for you are correct and there are no last chances on 6101's that I have seen out of the hundreds I have done so far. You should be seeing "phone restrictions on" and nothing more. You can try the code below if you wish.

I have tried one more code and it said "code error" and " cannot undo restriction".. now what I can do?

Code error on a 6101 doesn't sound right. Are you sure it's not a 6010 instead? This would be the first 6101 I have ever seen that gives "code error" messages.
