The game could be from a different region. Like if you're in the US and have a game from Europe, your region code is NTSC and the region code from Europe is PAL. For the signal outputs for video, NTSC uses (I think) 50 Hz while PAL uses 60 Hz, resulting in the screen rolling around. There are fixes for this, but we need to know what kind of mod you have (modchip or Swap Magic). SM has a region selector and most modchips do too.

Does your PS2 say anything like Toxic OS or Matrix when your PS2 boots?

You've got a Matrix Infinity, awesome chip! You should upgrade your firmware though, go here for it: (thanks to Tokijin) http://www.infinitymod.com/cgi-bin/matrix/site.pl?page=news
Also, from Tokijin's ultimate FAQ:
83. Q. I have a modchip. I downloaded a PAL/NTSC game and my TV/PS2 is in the the other video format. How do I force video mode?
A. With Matrix Infinity firmware upgrade 1.68:
Hit triangle+circle at boot up. Adjust the video mode to NTSC. Leave the Y-fix on auto. You may need to adjust to + or - 20 on occasion. The auto setting should work well.

Yes it is an awesome chip. That DMS 4 isn't too shabby either. Do what he said. You want to change the video mode to whatever version PS2 you have. I.E. NTSC if your PS2 is NTSC, or PAL if your PS2 is PAL. That will force your game into the correct mode and center the screen perfectly. It would definitely be a good idea to update your firmware using the link Ofnir provided.

That's all it's supposed to when upgrading the firmware. There's two possiblities here: A. It was a bad flash or you didn't burn the CD Right. B. You have an Infinity Clone like the modbo. The MI has a lot of clones unfortunately. What site did you order your modchip from?
Straight from the "readme" for Inf firmware v.1.68. I use v.1.68 because I customized my boot logo so that it says "Steelers" at boot up. :)
To Burn it to disc. Unzip the firmware.
1. Open Alcohol 120%.
2. Insert your blank CD-R.
3. Go to File--> Open. Browse to the cue file. Double click it.
4. Once it's open under Alcohol 120%, right click it and go to image burning wizard. Set the write speed to 4x, leave the write method alone, the data type to PS2, and select your recorder, click next.
Straight from the readme file for MI firmware v.1.68:
- NOTE : You can always recover from a bad flash by entering recovery mode.
==== This will let you boot the upgrade disc to reflash your Infinity.
To enter recovery mode:
V1-V12 and V14 PAL :
====================
Hold the reset button until the blue eject light turns on.
V14+ USA and JAP
================
USA : Hold reset until blue eject lights turn on, wait 3 seconds and
tap reset again.
JAP : Hold reset until blue eject lights turn on, wait 3 seconds and
tap reset, wait 3 more seconds and tap reset again.

I forgot to ask you this way earlier, Tokijin, do you know if you can use the triangle and cirlce at startup on an MI clone?

I'm not sure. Probably not. As far as I understand the Infinity clones worked based off the MI's firmware. The modbo is one example of a well known MI clone. Now they don't market themselves as MI clones, since everybody knows about them. The MX-2 chips were faked quite a bit as well. The modbo claims to have improved the MI's firmware and uses a modified version of it now. What a joke, the Mobbo is a POS chip. The MI, DMS 4, or CC are the way to go.

Well I checked out that site. I can't be sure, but it reeks of clone chip. Under PS2 modchips it doesn't specify what chip he installs. Maybe it does when you order one, I didn't check that far. I couldn't find a return policy. He shows a picture of a Matrix Infinity. If pressing triange+circle at bootup doesn't bring up the firmware menu, I don't think you have a legit Matrix. :( Most modders won't include games already on loaded a HD, at least no legit modders I know of. That could get him in trouble if anybody took the time to notice his site. He's also not on the official Matrix reseller list. Of course a lot of legit mod sites I know of aren't either. Sorry buddy, I think you got a clone. That's what all the evidence points to. :(
You could try patching the game to NTSC
37. Q. I downloaded a PAL game, how do I convert it to NTSC?
A. Use Adrenaline patcher if it's an EA game: http://www.megagames.com/ps2/ps2_utils_files.shtml#ADRenalin%20Patcher Note: I've been told this works for non EA games as well. The screen won't be perfectly centered, unless you have a modchip that forces video mode.
Or patch with PAL2NTSC patcher with Y fix for non EA games: http://forums.afterdawn.com/thread_view.cfm/121718
