It should look however you see fit. For example, I have a DS and GBA folder on my miniSD, and from the EZIV menu I just select whatever folder I want from the file explorer built into the loader.

Just make sure that when you're updating, you have the update file where it belongs, or else you won't be able to update properly.

Some flash cards require some file to be kept in certain places, like the M3 for example needs the folders NDSSAVE and GAMESAVE to store DS and GBA saves. And some homebrew and update files will require to be placed in specific folders for them to work.

Other than that, you can put games etc. wherever you want.

I have the following on my M3CF card:
BACKING (for different themes, must be called this to work on my device)
Gameboy (where I put my Gameboy ROMs)
GAMESAVE(for GBA saves, must be called this to work on my device)
GBA (to put my GBA ROMs)
Homebrew(for all my homebrew)
linux (for my Linux files, this is required by the homebrew app)
Music (For my MP3's)
NDS (For my NDS games)
NDSSAVE (for NDS saves, must be called this for my device to work)
NES (for my NES games)
Shell (Files for Moonshell to work, must be placed here)
System (Files for M3's Moonshell Extension to work, must be placed here)
Video (for my .dpg video files)

And the file structure for my G6 Lite is:
BACKING (for themes, must be here to work)
EMU (for the emulator file of the device, must be here to work)
PDA (for the PDA functions of the device, must be here for it to work)
GBA (for GBA games and GBA saves, must be here for GBA to work)
NDS (for DS games and DS saves, must be here for DS to work)
APPS (for homebrew applications)
GAMES (for homebrew games)
