Unable to Recover TOC. Failed at 100%

Guys pls I really need your help.. Everytime I burn my DVDs, almost 95% of the time I get the error "Unable to Recover TOC" and then "Cannot perform end of disc-at-once"... I tried cleaning my drive.. I even tried using 2.4x speed eventhough before, 8x works for me.. Also, after reaching the 100%, the "completed" progress bar in my Nero goes back to 0%... and then progresses until 14% and then moves to 100% before it displays the error... (To summarize things, it arrives at 100% twice before ending the session.. ) Here is my error log..
I would really appreciate it if someone could help me.. please..

First of all, thank you so much gwendolin for your reply.. I'll try reinstalling my Nero..
but I would like to ask further if my burner has other problems apart from what you have mentioned. I tried using a TDK DVD and the burning was successful (I'm not sure whether I just got lucky that's why it burned successfully).. But I noticed that the progress bar for the completed arrived at 100% twice.. Meaning after completing a 100% cycle, it returned to 0% and started again.. (I'm not verifying my discs).. But upon reaching 10%+, it quickly goes to 100% and the burning is finished.. This has never happened to me before..
Please enlighten me with the situation more.. But again, thank you so much for replying.. I really appreciate it

I dont think it's burner problems you are experiencing...more so Nero.
