Problems with recode - background hum and failed recodes

First problem is that some of my recodes have a low hum noise in the background. Sometimes it's so loud that it rivals the soundtrack. Sometimes it's much quieter, but still obviously not part of the movie. Sometimes I'll have a successful recode that doesn't have this hum noise present.

Second problem is that some of my recodes result in a video file that is far smaller than it should be and only contains a small portion of the source material. Recode still looks like it's working hard for a long period of time and doesn't give an error message.

It isn't dependent on the file being converted either. I might have a failed conversion and if I redo it, it might work the next time.

I'm using the HDTV - AVC profile and version 2.5.5.0 of Recode.
