Your RAM (256MB) may not be enough even for XP. Though if you have been copying without trouble, it must not be a cause.

?KEY_MEDIUM_ERROR? suggests a bad media. Though unless the disc is from new batch (package), it may not be a cause either.

Unless you have made some change on your PC, like new software installation, updates, and/or hardware changes, and have been copying without troubles, it is possible your burner has a problem?

Your Firmware is up-to-date (2005)
It might be time for a new burner.
and upgraded memory.
You could try to burn with Imgburn.
Burn at 4X.
You Could also try using a Cheaper Brand of Blank Media like (Oh God) Memorex.

Also Edit Out Your Serial #, It Begins with 1A22, Donnie
To Sum it up AGE has Caught up with your Burner.
