Im pretty sure that the subtitles stream is selected as a default with smartripper so if you rip using default settings then youll have the subtitles. You wont be able to use them unless you use a program like vobsub or subrip to recreate them. Here are a few guides you could try:

http://www.dvdrhelp.com/guides?tools=&madeby=&formatconversionselect=&osselect=&howtoselect=5%3B20&search=Search+or+List+Guides

Another method you can try is to find the movie's subtitles on the net and then use those with your movie. Youll need a subtitle filter like vobsub but that only works with subtitles created by vobsub (.sub files). You could use the ffdhsow filter to decode your subtitles, which is to me the best method because it reads many subtitle types which include .ssa, .sub, and .srt.

You can find a couple of sites to download subtitles here:
http://www.freestuffplaza.com/subtitles.htm

Good Luck.

Rip the VOB files using default settings.
Ok use this guide to create the subtitles with VobSub.

http://www.nanomessiah.com/subtitles.html

This method will 'burn' the subs to the movie which means they'll be permanent.

Yes its possible. But the file wont be burned into the image. I believe, and correct me if im wrong, that they call this soft subbing, where the subs arent permanent but are displayed when using a subtitle filter, which in your case is DirectVobSub.

Having read your post, you have already created the .sub /.ifo /.idx files. So all you need to do is to rename those files with the name of your movie. So if your movie is "matrix.avi" you would rename your files, matrix.sub /matrix.ifo / matrix.idx. Then place these files in the same directory as the movie and then double click on the movie. A window will pop up called languages. Right clicking in the window will bring up a popup menu where there's an entry called languages and you can turn the subs on or off.

If my post doesnt make any sense at all :-), read this:
http://www.doom9.org/dvobsub.htm

Also another filter you can use to display the subtitles is the FFDShow decoder filter.

Good Luck.

Im afraid Ive never run into that problem before, and cant offer much help, but ill try and see if i can figure out a solution.

Question:

How are the subtitles desynched? Do they come before the dialogue or after?

OK if you're still using vobsub, there is an option to offset the vobs, hopefully it will help you with your problem.
When you choose the vobsub filter change the Time Offset Option to "6000" since you said the subs were 6 seconds late.
