The response assumes that the PAL movie is available to rip the subs to sub/idx format.
The ripped subs are adjusted to the new frame rate and converted to .sup format.
(If the PAL subs were downloaded and are in a text form (.srt format) then 'Subtitle Creator' can be used to adjust the frame rate and convert to .sup format.)
In either case, the NTSC movie has to be demuxed to elementary streams then muxed along with the converted .sup file to create the new movie.
Starting from scratch, you would have a commercial NTSC movie that has to be ripped (main title - no menus, extras, etc)to the hard drive.
The ripped movie is 'shrinked' to fit a standard DVD disk.
Demux the shrinked files to video and audio streams.
Convert the PAL subs to the new frame rate and convert to .sup format.
Mux the video, audio and subtitles to create a new movie.
The software required to do this.
Rip movie, 'DVDFab HD Decrypter'
Shrink files, 'DVD Shrink'
Demux/Mux files, 'Rejig'.
Depending on the format of the new subtitles:
VOBSub (VOBSub-Configure and Sub Cutter included)
Sub2Sup
Subtitle Creator
It's less complicated than it looks and the time required varies on how many steps that you need to take;example ripping and shrinking the movie consume most of the time.

Created a short (ninety seconds) PAL movie (with subs) and an NTSC movie with a menu, but no subs.
Source was a 25FPS AVI file with matching .srt subtitles.
Demuxed the PAL subs from the new PAL DVD and converted them for use with the NTSC DVD and kept a functioning menu.

Was using English subs on an English audio movie (for test purposes), and thought the timing slightly off - but that may not be noticeable on a regular movie.
Fixed the perceived timing problem by using 'Subtitle Workshop'
Also found that that the subs had to be adjusted as to colour (used DVD Subedit to fix that).
The new files were tested with VLC to prove the menu was ok, then burned to DVD-RW and tested ok in a standalone player.

I can't do it. I don't know what happens.
I demuxed like the guide says but then when I use Subtitles creator to load the IFO, there is no IFO.
When I try to load the demuxed SUBS, there are none.
I don't get what I'm doing wrong.

Ok here's what happens. I demux and when I try to open the IFO from Subtitle creator it doesn't show anything in that folder. The IFO is there because I can see it when I open the folder in windows.

To go over the steps so far.
Used VobSub Configure to save the idx/sub file from the PAL movie.
Used Sub Cutter to change the frame rate from 25 to 23.976.
Demuxed the NTSC movie with Pgcdemux (to the 'demuxed' folder)
Subresync - 'Tools' => 'Manipulate SUP or VOBSub file'
Open IFO => VTS_01_0.IFO' from the PAL DVD (the one that has the subtitles)
This is where it refuses to open the original PAL 'VIDEO_TS' folder to let you at the .IFO file??

That guide assumes that you have a .srt subtitle file.
You cannot use the steps in part 3 or part 4, because you don't have a .srt file.
You have to use the steps shown in my first post to get the subs from the PAL disk and change the frame rate.
These subs will be idx/sub files.

Demux the NTSC movie to the 'Demuxed' folder with Pgcdemux
Open Subresync, click 'Tools' => 'Manipulate SUP or VOB SUB'
Click 'Open Sub' and load the idx file.
Click 'Open IFO' and open VTS_01_0.IFO from the original folder where you ripped the NTSC DVD files (not the 'Demuxed' folder).
Use the slider to get the subtitles in position on Ashly.
The reason you use the IFO is to get the correct screen size and colour information for the subtitles.
Unfortunately the NTSC movie has no subs - so the colours will be wrong - but you can't use the PAL IFO - because the screen size is different.
Ignore the colour for now and click 'Save SUB'.
Make sure you save it in .sup format.
Copy the .sup file to the 'Demuxed folder'
You then proceed to step 5 (Muxman) and remux the files from the 'Demuxed' folder using the new .sup file.

The thing is I downloaded the PAL subtitles from the net and they seem to be in srt. already.

Open the the .srt files in Subtitle Creator.
The default subtitle colours will be active.
Then (since these are PAL subs that were downloaded) 'Synchronize' => 'Convert Frame Rate' => '25 to 23.976' => 'Apply'
Ensure that 'Format' => 16:9
Adjust the placing of the subs and save them in .sup format to be used with Muxman..
'File' => 'Save SUP'.

Ok here is what I did. I took antoher dvd, decrypted it and demuxed it. For this dvd there were two files called subpictures which are the subtitles.
But for the first movie that I demuxed I could not find the subtitles for the life of me.

Another thing I forgot to say. I went ahead and followed the rest of the steps and used DVDsubedit to make sure the subtitles are fine but the timing is way off.
Not only that but when checking it with DVDsubedit I could see the original subtitles and the subtitles that I added at the same time on top of each other.

Thanks for your help but I just figured out what happened.
The movie is Downfall and the subtitles are automatic.
They are not optional so they can not be turned on or off.

I assume that you used a .srt file, changed the frame rate, converted to .sup and muxed a new DVD - but the subs aren't synced.
Run 'Subtitle Workshop'.
Click 'Movie' and load the new muxed DVD movie (the first VOB) then Click 'File' and load the .srt file.
Start the movie and immediately pause and note the time shown on the right side of the screen when the actor starts to speak.
Refine this as close as possible.
The first subtitle should have the same time.
Highlight the first sub to be changed, scroll to last and Shift-click on it.
All the subtitles should be highlighted.
Press the 'Move Subtitle' button (it has two right pointing arrows.
The time for all subs will be adjusted.
When satisfied, save the .srt file (SubRip).
Load the new .srt file into Subtitle Creator and save as a new .sup file.
Use this file in Muxman.

One thing about Workshop is whenever I rewind the whole time of the video changes. Kinda wierd.

Well I officially give up.
I'm sure someone out that can do this but I have invested enough time and failed.
When using Subtitle Workshop I find that I have to constantly be adjusting the subtitles. The real problem is that when ever I rewind.
Let's say I start from the beginning of the movie and it's at the 1:30 mark and I rewind it to 1:00 it'll show the credits instead of what's really supposed to be showing, which is the actor talking.
When I fastforward the same thing happens. It throws the time out of wack for some strange reason. So if I want to edit I have to stop and play the movie from the very beginning every time.
You can see how it's a major inconvenience.

I think that's because it cannot handle multiple VOB's, but as long as the time difference is constant, then all the subs should move by the same amount.

Another thing.
The original subtitles stop at 2:25 but these stop at 2:22.
I don't know if that is due to the framerate of PAL.

Update.
I burned the DVD but there is no sound. I can't activate the subtitles.
Also there are no chapters so when I go to scene selection I can not skip to a certain scene.
I did used IFOedit to pick the language but for some reason German is not on the list.
I'm not sure why the subtitles weren't there though.
I'll try again tommorow.

I ran into your problem of synchronising the PAL subs to the NTSC VOB files with Subtitle Workshop when I ripped a commercial DVD (it worked ok with my own burned disks).I don't know what's causing it - there are a number of fixes listed in Google, but I cannot get them to work.
The problem with chapters;did you extract the celltimes with Pgcdemux step and add them in Muxman.

You're right I had not put in the celltimes.
I tried again and the subtitles would not show up for some reason.
Even though you could choose them they wouldn't display onscreen. They were there when I checked with DVDsubedit.
I think I did something wrong with IFOedit.
I tried to burn a disc and forget about the menus and that did work fine.
Thanks for all the help.
I'll try again so I can keep the menus later on.
