PS2 Slim w/ Swap Magic

I am having an issue getting the DVD Boot disk to work on my system.

Factory DVD plays fine
Factory CD (don't have any)
Swap Magic Boot CD works fine
Swap Magic Boot DVD will not work at all.

The DVD boot disk used to work just fine, just quit one day a year ago. I've ordered several sets of boot disk since and none of the DVD's work. I also ordered the little plastic pieces (this latest pack came with an extra piece than before) but it didn't make a difference.

Does anybody know what is wrong, or what I can do to fix it?
