For those of you who had problems using with jjax's guide to loading backups, or for those who want a working Memory Card Exploit for their slims, here's how to make a working exploit to boot from the Memory Card, the actual CD-ROM, or tdude's method, through a USB Memory Stick.
CD method
1) First off, you need the following things:
-Action Replay MAX(V 3.36 was used)
-MAX Media Creator(included with Action Replay MAX)
-1 blank CD-R(Taiyo Yuden, Verbatim, or Sony)
-CD Loader ELF(found here: http://www.geocities.com/jjax1975/CDLOADER.zip)
-Cog-Swap ELF(link is down, will find a new one)
-PS2 Exploit Utility 5.1(found here: http://www.iscentral.net/console/PS2_Exploit_Utility5.1.zip)
-WinRAR(to extract the compressed files)
-Alcohol 120%(to burn the exploit disc)
-USB Memory Stick(to put CD Loader)
2) Now that you have all you need, un-zip all the files you downladed with WinRAR and launch PS2 Exploit Utility.
3) Here's what you have to do for each of the tabs:
-Title.db: nothing
-Loader: nothing(keep uLaunchELF 3.61 selected)
-Files: click "Browse for file", locate COGSWAP.elf, click it, and click "Add"
-IP Options: click "Yes" ONLY if you will use your PlayStation 2 over a network
-Region: NTSC for NTSC U/C consoles, PAL for PAL, and JAP for NTSC J
-Extras: keep the dummy file the way it is and assign Cog-Swap to one of the buttons
-Create Exploit: select "CD/HD image (bin/cue)" and select "Live CD" in "Options for CD image"
4) Now that everything is set up, click "Create Exploit!". You will get a window saying that there are no title IDs in Title.db. Click "Yes". When the job is done, you will see this message:
A CD image has been built here: C:\IsCentral\ that can be used as a CD or HD image!
5) When the image file is made, put in your blank CD-R and start Alcohol 120%. Click "Image Burning Wizard". Locate your CD image, click it, then click "Next". You will now be at a window called: Prepare your CD/DVD recorder. Set the write speed to 4X, datatype to PlayStation 2, check "Rectify Sub-Channel Data", then click "Start".
6) You now have the Exploit CD ready. You now need a way to boot it. We'll use CD Loader. Install MAX Media Creator and start it up. Click "Add file", and locate CDLOADER.elf. Click "USB Drives" in "Output Media", select the location of your USB memory stick, and click "Create".
7) You now have CD Loader and your Exploit disc. Put Action Replay MAX in your PlayStation 2 and power it on. When it's loaded, look for MAX Media Center(called MAX Media or Media Player), select it and press X.
8) Once it's loaded, plug in your USB Memory Stick and wait until CDLOADER.elf is displayed on the top right of the screen. If you get a message saying that nothing was found on the USB Memory Stick, keep plugging and unplugging it until CDLOADER.elf shows up. Once it does, highlight "Play" and press X.
9) When CD Loader is loaded, swap the Action Replay MAX disc for your Exploit boot disc and press X. If you get a screen that becomes red with a pink border turning to yellow and disappearing, the boot is a sucess, but if the border stays pink, you did something wrong.
10) You should now see LaunchELF's main menu. If you get an error message while trying to boot Cog-Swap, press "Select", locate the button you assigned Cog-Swap to, press O, press O on Cdfs, press O again on "BxDATA", find COGSWAP.ELF, and select it with O. Return to the main menu, and press the button assigned to Cog-Swap.
11) Here's how to use Cog-Swap:
CD-ROM:
1) Swap the uLaunchELF disc with your backup when the Cog-Swap screen loads with a blue disc and the writing "PS2 CD-ROM" or similar underneath.
DVD-ROM:
1) Remove the piece you have put over the front lid sensor of your PlayStation 2. The piece should be over this thing:
2) Insert a high TOC ORIGINAL DVD-ROM game(one that has a silver base) whose size is over 4GB(you have to check on your computer, and you CANNOT use a Dual Layer DVD-ROM game, which are those that are over 4.37GB).
3) Place the piece you removed from the lid sensor shown earlier back where it was.
4) Keep an eye on your screen until a yellow disc shows on the screen with "PS2 DVD-ROM" or something like that under it.
5) Once you see the yellow disc with "PS2 DVD-ROM" or similar, press X on your controller. The disc will spin again(it stops when the screen shows the yellow disc). When the disc spins, keep your eyes on your TV screen until you see the yellow disc turn purple and "PS2 DVD-ROM" or similar kind of writing change to "Use Cogswap".
6) When that change happens, swap your original with your backup.
Memory Card method
What you need:

With that, click "Edit", "Add File", locate COGSWAP.ELF, and select it. You should now see COGSWAP.ELF as part of the list of files in the save. Now, change the Root/ID to BADATA-SYSTEM for NTSC U/C PlayStation 2s, BEDATA-SYSTEM for PAL, and BIDATA-SYSTEM for NTSC J. Save it as "exploit.max".
7) Plug your USB memory stick in one of your computer's USB ports. Locate "exploit.max", right click, copy. Browse to your USB memory stick, and paste the file there.
8) Now that you have everything you need, it's time to install the exploit. Turn on your PlayStation 2 with Action Replay MAX. Once loaded, select "MAX Memory". When that is loaded, go to "My devices", select your Memory Card, and delete "Your System Configuration". When that's done, go back and select your memory stick. Select "NEW_FILE" and uncrush it to your Memory Card. Once that's done, your exploit is installed.
9) Return to Action Replay MAX main menu. This time, choose MAX Media Player(called MAX Media or Media Player) and press X.
10) Once it's loaded, plug in your USB Memory Stick. Wait for CDLOADER.elf to show up(it will sometimes say that there's nothing in the USB Memory Stick. Unplug it and plug it back into another port until you see CDLOADER.elf on the screen) Highlight "Play" and press X.
11) When CD Loader is loaded, the Action Replay MAX disc will stop spinning. Swap it for your Exploit boot disc and press X. If you get a screen that becomes red with a pink border turning to yellow and disappearing, the boot is a sucess, but if the border stays pink, you did something wrong.
12) You should now see LaunchELF's main menu. To assign Cog-Swap to a button, press "Select", press O on the button you wish to use to boot Cog-Swap, press O on mc0(or mc1 if your Memory Card is in slot 2), press O again on "BxDATA-SYSTEM", find COGSWAP.ELF, and select it with O. Return to the main menu, and press the button assigned to Cog-Swap.

Now we're ready to boot our backup! Here's how to boot it(instructions written by tdude511 and editied by xXGhostXx):
1) Start cdloader.elf from the memory stick in Max Media Player
2) When the AR disk stops spinning, swap it for the uLaunchElf disk, the one you created with the utility and alcohol and press X.
3) uLaunchElf should load. If it didn't, then one of three things are wrong, 1. You burnt the media wrong, 2. Your laser may need cleaning, or 3. You're using crap media(Memorex and the like).
4) Press select to access the options/config.
5) Scroll down to any button you want. This button will load cogswap.elf from your memory stick when you press it at the main menu. Press O.
6) There should be about 6 or 7 different drives listed here, like both memory cards, etc. Scroll to one that sounds most like your memory stick, mine said "mass" or something like that, it might be that for everyone but idk. Press O.
7) Scroll down and select cogswap.elf and press O.
8) Scroll down to "OK" and press O.
9) Okay, we're down to final step! You should now be back at the main menu. Press the key you assigned to load Cog-Swap. And Voila!!! Cog-Swap will load!
Here's how to use Cog-Swap:

This was tested on a Version 14 slim. If you have any questions(including people who have fat PlayStation 2s), feel free to ask. I'll be glad to take your questions.
DISCLAIMER(proposed by Ghost): This guide is only intended for people who make LEGITIMITE backups of their games. I am NOT responsible for people who are using this guide for wrongdoing(piracy).
Update: Edited the USB Memory Stick method using a proposed layout by xXGhostXx and did a slight edit on the use of CD Loader in the CD and Memory Card methods.

Okay, but, how do you boot from the Memory Card? Since the PS1 exploit has been fixed in the slimline PS2s, do you have to load AR Max, to CD loader, to LaunchELF?

I've had the same problem, I load up uLaunchElf 3.61 fine, but when I try and launch Cogswap, it says "File not found in BADATA-SYSEM," which I believe is the directory of the memory card. Are there any launchers that can launch Cogswaploader from a usb key instead?
EDIT: Never mind, I got it to work =))))) Finally I can play backups...expect a tutorial soon! =)

Monke320- It's actually from Action Replay MAX to MAX Media Center to CD Loader to LaunchELF. I have edited the guide for a method to make a workable Memory Card Exploit which will use the disc with LaunchELF booted through CD Loader as the trigger disc. You need a USB memory stick to install the exploit using MAX Memory.

Actually, I did some things different. Instead of wasting more CDs, you can just put CDLOADER.ELF on a memory stick instead. I also put COGSWAP.ELF on the memory stick too. Once I got to uLaunchElf 3.61 I loaded cogswap from my usb key. Maybe you could add this to your post? [With credits of course =)] Here are the instructions:
1) Start cdloader.elf from the memory stick in Max Media Player
2) When the AR disk stops spinning, swap it for the uLaunchElf disk, the one you created with the utility and alcohol and press X.
3) uLaunchElf should load. If it did not, you burnt the disk wrong.
4) Press select to access the options/config.
5) Scroll down to any button you want. This button will load cogswap.elf from your memory stick when you press it at the main menu. Press O.

I successfully backed up and played a copy of DDR MAX 2. I used SSX: On Tour for an original disc, very high TOC.

Hey, thanks, MasterChu. I just wanted to find a way to make a working exploit for slims since there aren't many around. So, tell me, did you try it, and what was the result?

Which method, CD or Memory Card? The CD method worked for me, and so did the Memory Card method(just a test, I removed it after). Did you follow the instructions for burning the exploit disc? Remember, burn with Alcohol 120% at 4X(or 1/4 the maximum disc speed), datatype set to PlayStation 2, and rectify the Sub-Channel data.
Are you using any of the 3 recommended media brands, Taiyo Yuden, Verbatim, or Sony? If not, then the problem is maybe your media.

This really needs to be stickyed. This is the perfect way to play backups on slims.

So...I swear to god i burned the uLaunchElf cd correctly, but cdloader is telling me that it is 'invalid media'. I wish I had more info, but I am just looking for ideas here. Thanks a lot!

When it says "Invalid media" on the screen(bottom-left corner), that's when you press Triangle to retract the laser and put in Action Replay MAX. "Invalid media" will change to "PS2 CD". That's when you swap for uLaunchELF. This should fix your problem.

You need a way to make your PlayStation 2 get a signal that the lid is always closed. Try my MOD for slims, see if that helps, and if you have a Version 12, post how you got the straw in(remember that the Version 12 has no metal plate in front of the clip near the hinge). Link in my sig.

Totally amazing Tutorial Ace_2, well done. I'm a relative newbie to PS2 modding and backup gaming but with the help of this tutorial I am able to run backups and also I added USBAdvance to the Exploit CD and can also run games from my USB External HDD perfectly.. and they run FAST too, no choppy movie scenes or sound like I've seen mentioned in other Forums. Thanx.
<runs off to PS2 to get back into playing Black>
BTW.. I have a basic (non-chipped) PS2 Slimline v13 with toggle-switch mod to bypass sensors (which I modded myself) :-p

waille- This will also work with Swap Magic, but this defeats Cog-Swap's purpose, as it basically does the same thing as Cog-Swap. Instead of Cog-Swap, you can put any program you want into PS2 Exploit Utility to burn on your CD-R.
jjax- Wow, man, thank you very much. I even read what you posted on your guide, that was a mention I was not expecting to see. Here's how we put this situation of fats and slims:

How does that sound? Would you say it deserves a stickey? I mean, this guide is getting a lot of positive comments, and it's maybe one of the few methods to get a backup-booting program other than Swap Magic to work.

Well i dont think it does defeat the purpose of cog swap, i want to use this to play ps1 back up on my slim ps2, and you can't do that with the swap magic
Oh and by the way your link "how to use cog-swap" is broken.

If you already have Swap Magic, what's the point in even doing this? Useless for Swap Magic users(unless a certain game can't boot or you want to put other ELFs on the disc). If you want to use this exploit guide to boot Cog-Swap so that you can get games not working with Swap Magic to work, or to use any other utility(USB Advance, for example, or HD Advance if you have one of the two IDE hard drive add-ons for slims), skip the step to make the CD Loader disc. Just boot Swap Magic CD and swap it with the exploit disc.

Hmmm .... why every time i run "PS2 EXPLOIT" executable im getting this error :

And it launches the browser & goes to their forum.
