To keep only the movie (no menu, extras, etc) click 'Re-author' then on the right pane, select the largest Title (usually Title 1)and drag it to the left pane and 'Backup'.

after selection is it I have to click "set start/end frames"?
May I know if I choose full disc instead of re-author, will the unwanted content likes unautorised ads be deleted?

If you elect to keep the full disk, you will lose the FBI warning - but keep the trailers, extras and menu.
If you wish to select the start/end frames, you must 'Re-author'.

If you have distortion in the video, there is something wrong with the disk - or perhaps you never ripped the disk using the likes of DVDFab HD Decrypter.
DVD Shrink should not be used to rip the original disk - use it only for shrinking/re-authoring.

If there are two titles under the 'Main Movie' then it's possible that you have two versions of the movie (perhaps a Directors Commentary).
If that was the case and you selected it (by dragging to the left pane) then clicked on the 'Compression Settings' tab on the right pane, you might see that the audio was a single AC-3 2 channel, whereas the title you want might have AC-3 5.1 in various languages, DTS audio and subtitles
